Request to forward your medical records to another doctor |
We have been requested to forward your complete medical file to another doctor
This practice follows the Code of practice for the Management of Health Information in Medical Practices developed by the Royal Australian College of General Practitioners. This means that your personal health information is kept private and secure. The approach used in Code is consistent with the provisions of Federal and State Privacy Legislation.
It is usual for our practice to send a health summary of relevant matters rather than your entire medical file. While we respect your right to decide how your personal health information is used, we wish to draw your attention to some issues you may not have fully considered.
- Most Doctors would prefer to receive a health summary.
- For legal reasons we are obliged to hold your original record at the Lennox Head Medical Centre.
- We are entitled to charge a fee for any reasonable costs associated with preparing a summary, collating the record, copying and posting the records and report.
If after considering these matters you decide you wish to proceed please complete this consent and return it to our practice. We undertake to provide the information to your doctor as quickly as possible.

The Australian Medical Association has a recommended fee for copying
If you request direct access to your full medical record your doctor may under rare circumstances need to consider the risk of any physical or mental harm to you or any other person which may result from disclosure of your health information. Under some circumstances the request may involve considerable time. Depending on what is involved, you may be asked to contribute to the cost of providing the information.
