We have been requested to forward your complete medical file to your solicitor.
This practice follows the Code of practice for the Management of Health Information in Medical Practices developed by the Royal Australian College of General Practitioners. This means that your personal health information is kept private and secure. The approach used in Code is consistent with the provisions of Federal and State Privacy Legislation.
It is usual for our practice to send a report to solicitors of relevant matters rather than your entire medical file. Your solicitor has requested your complete medical record. While we respect your right to decide how your personal health information is used, we wish to draw your attention to some issues you may not have fully considered.
- Do you know who in your solicitor’s office will have access to the file?
- Do you understand the purpose for which the information will be used?
- Do you understand the systems your solicitor will employ to maintain the privacy and security of your personal health information?
- Would you like the opportunity to review the record and determine if you are comfortable with the information that will be forwarded?
If after considering these matters you decide you wish to proceed please complete this consent and return it to our practice. We undertake to provide the information to your solicitor as quickly as possible.

If you request direct access to your full medical record your doctor may under rare circumstances need to consider the risk of any physical or mental harm to you or any other person which may result from disclosure of your health information. Under some circumstances the request may involve considerable time. Depending on what is involved, you may be asked to contribute to the cost of providing the information.
